Sacroiliac Joint Rhizotomy

A sacroiliac joint rhizotomy is an outpatient, minimally invasive procedure performed to provide long-lasting relief of back pain. Candidates for this procedure suffer with sacroiliac joint pain, and have only experienced temporary relief from intra-articular sacroiliac (si) injections with local anesthetic and steroids. Essentially, small sensory (pain) nerves of the si joint are disrupted and deadened, which in turn stops painful sensations from being sent to the spinal cord and brain.

Sacroiliac joint rhizotomy is a therapeutic procedure, which uses radiofrequency generated heat to disable pain fibers. Patients receive iv sedation and lay on their stomach for this procedure. Under direct fluoroscopic visualization (x-ray guidance), a thin needle is placed into the posterior capsule of the joint. After confirming correct placement, the radiofrequency lesioning occurs for 90 seconds. This is repeated at several sites within the joint. The procedure may be performed unilaterally or bilaterally. Patients are monitored for a short while after the procedure before being discharged to home.

In general, the risks are low. The most frequent complication is a post-procedural flare up of pain. This may last up to a week. Infection may occur, but the incidence is low. A sacroiliac joint radiofrequency rhizotomy may provide over a year of pain relief, allowing patients to reduce pain medications and increase their activity level.
